Supreme Care's service cover 18 districts:
Eastern District, Wan Chai, Central and Western District, Southern District, Kwun Tong, Wong Tai Sin, Sham Shui Po, Kowloon City, Yau Tsim Mong, Sha Tin, Tai Po, North District, Kwai Tsing, Tsuen Wan, Tuen Mun, Yuen Long, Sai Kung, Outlying Islands
Monday to Saturday 8:00 am to 6:00 pm
other times and public holiday, 20% surcharge
Service Process
Supreme Care discusses the service package agreement with the elders or family members participating in the community care service voucher pilot scheme

within one month to complete the assessment and develop an individual care plan
The case must be reviewed one month after the commencement of services within the Personal Nursing Care Program
↓
Case reassessment every three months
If the elderly who participate in the pilot project have special physical and mental changes, they must be followed up and re-evaluated immediately.
