Beneficiary
The Social Welfare Department (SWD)'s "Community Care Service Voucher for the Elderly" is a model of "money follows the person". Eligible elders can use the community care service voucher ( Community Vouchers) to choose appropriate community care services to support them in home care. The pilot scheme covers all 18 districts in Hong Kong and it has become permanent in 2023. The maximum number of community coupons can reach 12 000.
Recognized by the Social Welfare Department of Hong Kong, SUPREME CARE has become a designated service provider under the Community Care Service Voucher Scheme for the Elderly, and is legally entitled to provide corresponding care services for the elderly.

Eligibility
1 Elderly persons who are on the Social Welfare Dpartment Central Waiting List for Subsidized Community Care Services and/or Residential Care Services and have not received any Residential Care Services or Subsidized Community Care Services are eligible to apply community coupons.
2. The Social Welfare Department will send letters directly to eligible elders, inviting them to apply for community vouchers.
3. Elders who are interested in applying for community vouchers can submit the application form to the community voucher office. Under normal circumstances, it takes about two to four weeks from the approval to the notification of the application result.

Service Process
Application Method for the Elderly Community Care Service Voucher Scheme:
①Online Application:
Go to the Social Welfare Department's website( https://www.swd.gov.hk/en/pubsvc/elderly/cat_commcare/psccsv/index.html)
select the "Voucher Information System for the Elderly," and click on "Submission of Application Form ( Eligible Elderly Persons Only).
The Voucher Information System for the Elderly (VISE) is a web portal system manned by SWD. The eligible elderly persons being invited to join the scheme might submit application form, calculate CCSV Co-payment Category, real-time check their voucher usages, search for RSPs and confirm their service packages as well as download the monthly service timetable, etc., through VISE.
② Mail (Address:Rooms 2601-05A, 26/F, Two Chinachem Exchange Square, 338 King's Road, North Point, Hong Kong)
SWD will send invitation letters to the eligible elderly persons in batches according to their application dates on the CWL for subsidised LTC services, inviting them to join the Scheme. The eligible elderly persons who are invited can submit the completed application form together with the supporting documents to CCSV Office. They can also submit the online application via VISE.
In general, the SWD will complete the vetting and approval process and issue the vouchers within four weeks upon receipt of applications.
