Beneficiary

The Social Welfare Department (SWD) launched the third phase of the "Community Care Service Voucher for the Elderly" pilot project in October 2020. With the model of "money follows the person", eligible elders can use the community care service voucher ( Community Vouchers) to choose appropriate community care services to support them in home care. The pilot scheme covers all 18 districts in Hong Kong and the maximum number of community coupons can reach 8 000.

Eligibility

1     Elderly persons who are on the SWD Central Waiting List for Subsidized Long-term Care Services and/or Residential Care Services and have not received any Residential Care Services or Subsidized Community Care Services are eligible to apply community coupons.

2.    The SWD will send letters directly to eligible elders, inviting them to apply for community vouchers.

3.    Elders who are interested in applying for community vouchers can submit the application form to the community voucher office. Under normal circumstances, it takes about two to four weeks from the approval to the notification of the application result.

Notice

Due to the raging novel coronavirus epidemic, in order to ensure the health of customers and employees, the company must be notified in advance if any of the following situations are met before on-site nursing services:

  1. Have left Hong Kong in the past 14 days / within 14 days before receiving home care services, including by air, ship, etc.

  2. Visited high-risk areas such as hospitals or clinics in the past 14 days/14 days before receiving home care services

  3. Fever and symptoms of acute respiratory infection or pneumonia, such as fever, cough, chest pain and dyspnea, etc. in the past 14 days / 14 days before receiving home care services

  4. Had close contact with a confirmed case of novel coronavirus in the past 14 days / 14 days before receiving home care services when symptoms appeared

  5. There were confirmed cases of novel coronavirus in the building where you lived or worked in the past 14 days / 14 days before receiving home nursing service

The above are all measures that the company must take to ensure the health of customers and employees.
